SAD NEWS THE INDIAN VARIANT IN THE US : CORONAVIRUS

Indian hospitals have been overwhelmed by COVID-19 patients in recent weeks, after the Indian variant accelerated infections - and fatalities - across the country.

Weekly averages of hundreds of thousands of infections recently took India's case total above 25 million. And fatalities are approaching 280,000 - although analysts warn it is in fact far higher.

One case of a new COVID-19 variant first detected in India has now been found in the United States. Spotted first by researchers at Stanford University, the variant was found in Northern California.

The Variant in question has two (2) mutations - one similar to the variation found in the strain from California which was spotted in the spiky protein which the virus attaches itself to.

The Indian variant was found in a patient from the San Francisco Bay Area by Stanford's Clinical Virology Laboratory, as reported for example by NBC, citing a spokesperson from Stanford. This marks the first case of the variant in the United States.

The variant was first detected last month by Indian health officials. India had witnessed a slowdown of the spread of COVID-19 in the last few months of 2020. But in March 2021, cases continued to rise across the country, nearing 100,000 daily infections in April 2021.

Cases in California are currently declining after witnessing a surge in the winter months due to which cases increased in November running into the new year.

This relief in terms of numbers has prompted California authorities to begin reopening parts of the state, starting with its most populous county in Los Angeles.

Even then, California is the hardest-hit state in the US, recording more than 3.5 million known infections and 58,000 deaths, as per data from California's health department.

DRIVER NEGLIGENCE - TRAIN DRIVEN IN WRONG DIRECTION : KL LRT

KUALA LUMPUR, MALAYSIA:Preliminary investigations into the head-on collision between two (2) KL LRT trains has indicated that driver negligence was the cause of the accident last night, May 24, 2021, said Malaysian Transport Minister Wee Ka Siong on Tuesday (May 25, 2021).

The collision, which occurred near KLCC Tunnels station on Monday everning, May 24, 2021, and more than 210 injured passengers, with six (6) presently in critical condition.

In a press conference, the minister said: " Preliminary investigations indicate that the accident was the result of carelessness of the hostler who drove TR40 in the wrong direction. " The hostler was driving the faulty train to the depot.

" TR40 was supposed to travel southbound towards Dang Wangi, but it moved northbound, a different orientation. This resulted in the collision between TR40 and TR 81.

Dr Wee explained that TR40, which wasn't carrying any passengers, was heading southbound from Gombak to Subang, when it "suffered a failure" at Kampung Baru station. The failure occurred because the train had failed to operate automatically.

The central operations centre then instructed the driver of TR40 to drive the train manually from Kampung Baru station to Dang Wangi station, said Dr Wee.

However, at 2033 pm, the driver contacted central command to inform the latter that TR40 had collided with another train, which was later identified as TR81. There were 213 passengers on TR81.

Today, Dr Wee announced that a special task force has been formed to investigate the cause of the incident.

He added that the task force, which will be headed by the transport ministry's chief secretary, will be given two (2) weeks to present their findings on the incident.

The collision is the first accident in 23 years of KL LRT operations in Malaysia.

The Kelana Jaya Line operating company Prasarana Malaysia has outlined that out of the 213 passengers affected, 64 were still hospitalised at Hospital Kuala Lumpur.

Out of the 64 six (6) are in critical condition. Three (3) of them require ventilation support.

When asked during the press conference to comment on the financial losses incurred from the incident, Dr Wee said that it was too early to estimate since the authorities were still cleaning the debris and fixing the damaged track.

He added that this process will take "three days" but reassured commuters that train services will proceed.

On Tuesday morning, Prasarana Malaysia said that the LRT Kelana Jaya Line in Kuala Lumpur resumed service at 6 am with trains operating on a single track, adding that there will also be supporting shuttle bus services.

On Monday night, Malaysian Prime Minister Muhyiddin Yassin that he has instructed the Transport Ministry and Prasarana Malaysia to conduct a "full investigation to identify the cause of the accident". He added that "stern action will be taken immediately."

On 'Stupid' Policies & SOPs Of COVID-19

MP SPEAKS

In his public address yesterday on television (May 23, 2021) and through social media, Prime Minister Muhyiddin Yassin said that it was okay for the public to scold "Stupid Prime Minister".

I'm not sure if the PM was trying to score sympathy points with the public but I don't think this is an appropriate public response to the increasing number of COVID-19 cases in Malaysia. Please don't blame the public for being frustrated if the policies or standard operating procedures (SOPs) announced by certain ministers are seen as "STUPID".

For example, in the latest tightening of the movement control order (MCO) 3.0, the following three policied do not make sense to me and probably to the larger public as well.

Firstly, the restriction on businesses including eateries to operate only until 8 pm at night. This will have the effect of making people crowd at the restaurants and mamak stores which are doing take away close to the closing time of 8 pm. At the same time, it will also endanger the food deliverers on motocycles (Food Panda and Grab Food, just to name two of the more popular food delivery options) as they rush to fulfill the final orders nearing 8 pm. Why not extend the operating hours of restaurants for the take out option until 10 pm so that such a rush does not occur just before 8 pm? Pardon my language, this is a really stupid policy or SOP.

Secondly, reducing the capacity of public transportation by 50 percent will increase the crowding effect of those taking public transport. This will increase the probability of COVID-19 spreading, especially if physical distancing is not possible in crowded public transportation options such as the LRT, MRT and buses especially in the Klang Valley.

Thirdly, reducing the civil service capacity to just 20 percent will likely increase the queues and waiting lines for those who have no choice but to go to government offices to process documents, pay fees and other government related transactions, especially if they don't have the capacity to do all of these things online!

Why The New COVID-19 Variant Is Spreading Fast ?

The current wave of COVID-19 Pandemic has changed all the dynamics that were experienced last year 2020. The number of patient is more, and people are getting infected rapidly.

Reported by Specialist in Internal Medicine at Apollo Hospital Muscat, Dr Dilip Singhvi, noted that the steep rise in numbers is accompanied by complications increasing in cases ending in mortality.

One of the reasons for this, according to Singhvi, "is the virus' spread, that is 30% faster than last time, and it is 70% deadlier, which explains the rise in death. The world has experienced double mutants, and now we have noticed triple mutants reported in India.

Have the symptoms changed ?

Singhvi explained that the main symptoms that were observed with the coronavirus at the beginning are still prevalent. "Fever, headache and body ache, muscle pain, cough, feeling or tiredness, fatigue, and the feeling of drained out, all continue to be present. The new symptoms were are noticing are the cases of loose motions, diarrhoea, abdominal pain, and rash, and out-of-proportion muscle cramps. These are the new symptoms that have come with the new variant," he explained.

Another important factor is the change in age group. " This variant is now involving more with the adolescent and younger are group than the older virus that was infecting the elderly population more.

And this is why there is a massive increase in infection because the younger population is more mobile. The working population is also younger who are on the move for work, entertainment, etc. Children are also being infected more now.

It is not just symptoms that have increased; the complications due to the COVID-19 pandemic infection are taking a new turn.

" As you know COVID-19 is a spectrum. It is a dynamic disease. We have a spectrum of asymptomatic to mild, mild to moderate, moderate to severe, and very severe diseases. So there are five categories of patients.

Usually asymptomatic patients do not need treatment, only isolation at home; even the mild symptoms can be treated at home or as an outpatient or through telemedicine.

They do not require hospital admissions at the most all they need is a visit to the clinic. With other categories such as mild to moderate and severe, these patients will require admission for hospital stay treatment," he noted.

COVID-19 treatment can be expensive. The treatment cost can vary due to the complications which were not widely seen last year 2020.

" There is an increase in patients in the category of severe cases. That is why hospital admissions have increased. In the last four to five weeks, the numbers have steeply gone up, and the number of hospital admissions has crossed last year's high, as is the case with ICU occupation. Deaths are more. The end result is the cost of treatment going high," he said.

This time doctors came more cases of clot formations in the brain, heart, lungs, legs, lack of circulations, involvement of kidney, and liver as the urine output decreases.

The liver enzymes tend to go high, which are some of the reasons patients stay longer in hospitals.

The cost goes higher with the severity of the case, and the care one might need. A mild to moderate case might cost a few hundred Omani Rials, but one the patient becomes moderate to severe, the oxygen requirement might go up.

The major costs are oxygen, ventilator and the injections, and other required services the team that is assigned to the treatment.

" The patient would have to be investigated more extensively and would have to be repeated depending on the severity. If mild cases require two to six litres of oxygen in the second category patients might require 8 to 10 to 12 litres per minute. The severe cases would require the next stage, which is ventilation that could be invasive or noninvasive, this would also increase the ICU stay," he pointed out.

The length of stay depends on the severity and how quickly the patient recovers.

" During this period, we have noticed that people come with severe situations that need a ventilator immediately or within 48 hours."

One crucial factor, everyone should be aware of is the current variant is that it brings out the same symptoms of seasonal flu.

" I request all people not to be denial and not to take the symptoms lightly. Take consultation with your own doctor because it could be COVID-19, and be aware that you could have transmitted it to others. What I would suggest is that if one develops any of the symptoms, fever, headache,  body pain, weakness, tiredness etc please isolate yourself so that you do not pass it on to other family members and do not go to work so that you do not give it to your colleagues. Try to get to a medical facility and get yourself tested.

The test will give us either peace of mind if the test proves you are negative or give you the correct diagnosis for treatment.

This way, we can control the spread of the disease. Many patients are in the mode of denial and are not ready to do the test. This attitude needs to be changed because this would help control the spread of the disease, especially amongst the blue collared workers or employees who work in clusters. If the infected person does not get investigated on time he could give it to others in the camp same in an office environment," said Dr Singhvi adding that is important for people to know the symptoms because it is best to control it while it is still at mild stage as it controls the spread of the disease as well as the spread of the infection.

POST-COVID 19 COMPLICATIONS

Many patients are also undergoing post-COVID-19 complications such as what medical practitioners call as long COVID-19 syndrome. They continue to have shortness of breath, chest discomfort, body ache, and tiredness.

" Many people are going through prolonged weakness post isolation and post-COVID-19 period with some carrying low-grad fever. All of this at times, continues for two to three months. People also tend to have problems in concentration and remembering things correctly as part of the post-COVID-19 syndrome. The best thing is to talk to your doctor immediately, practice relaxation and meditation techniques to bring back the focus. Usually, the focus will come back, but it takes time, sometimes up to three months or longer than that. So do not ignore these symptoms as many patients are also experiencing palpitations, anxiety, and disturbed sleep. All of these symptoms are mild and can be brought under control," advised the doctor.

NEELOFA FAILED ABOUT SOPs IN FINAL STAGE - COVID -19

SEREMBAN, NEGERI SEMBILAN :

The police are in the final stage of completing the investigation papers (IP) on celebrity Neelofa Mohd Noor and husband, Haris Ismail, for allegedly violating the standard operating procedures on the Conditional Movement Order (CMCO).

Negeri Sembilan police chief Datuk Mohamad Mat Yusof said the IP had been referred to the prosecution, but returned to the police for further investigation.

" There are some actions that need to be taken and we expect a decision on it to be made soon," he added at the wearing of rank for Sergeant Mat Shah Kasim early this month.

Investigations was conducted after photos and video clip of the couple shopping at a carpet shop went viral from Neelofa Insta recently.

The couple was called to the Nilai Police station last Tuesday to give their statement.

Meanwhile, Mat Shah, 57, was conferred the rank of a sergeant for this bravery in saving two victims, a senior citizen and her son, who is a person with disabilities (PwD), in a fire in Kampung Seberang Batu Hampar, Rembau, last March 04, 2021.

The policeman was on his way home from work when he saw the fire, and without hesitation, rushed to help after watching a woman running back into the burning house to save her son.

Sehingga jam 2 petang 21 Mei 2021, hampir 60,000 orang menandatangani petisyen bertajuk Langgar SOP : PDRM, Penjarakkan Neelofa dan Suaminya Segera RM 1 Juta Sanggup kah ?
